You may file a complaint with KAM Health or with the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated.

KAM Health will not retaliate against you for filing a privacy complaint.

Our Responsibilities

KAM Health is required to:

  • Maintain the privacy and security of your protected health information.

  • Provide you with this Notice describing our legal duties and privacy practices.

  • Follow the privacy practices described in this Notice.

  • Notify affected individuals when a breach occurs that compromises the privacy or security of their protected health information, as required by law.

  • Provide you with a copy of this Notice upon request.

  • Obtain your written authorization for uses and disclosures when required by law.

KAM Health reserves the right to revise this Notice. If our privacy practices materially change, we will update the Notice and make the revised version available as required by law.
